In-vitro wear of fixed and mobile bearing unicompartmental knee replacements

چکیده

Introduction Unicompartmental knee replacement (UKR) is an increasingly popular surgical intervention for the treatment of osteoarthritis, involving the replacement of only one compartment of the natural knee. There is promising clinical literature which shows excellent long term survivorship of both fixed and mobile bearing UKR, with fixed bearings showing more consistent long-term performance [1]. There is limited data examining the in-vitro performance of UKR [2]. The purpose of this study was to investigate the influence of bearing design and kinematics on the in-vitro wear performance of medial and lateral unicompartmental knees.

Effect of Polyethylene Crosslinking and Bearing Design on Wear of Unicompartmental Arthroplasty.

Wear and polyethylene damage continue to be important factors affecting outcomes of unicompartmental knee arthroplasty. We compared two design rationales for unicompartmental arthroplasty: fully congruent mobile bearings; or moderately conforming fixed bearings using experimental and computational wear simulation. Experimental wear rates were 3.89 (±0.12) mg/million cycles for the highly crossl...

Mobile or fixed unicompartmental knee prostheses? In-vitro wear assessments to solve this dilemma.

The unicompartmental knee prosthesis is an attractive alternative to total knee arthroplasty. Current UKP devices can be subdivided into two groups based on different design principles: fixed bearing knees, where the ultra-high molecular weight polyethylene meniscal component snap or press fits into the tibial tray, and mobile bearing designs which facilitate movement of the insert relative to ...

Increase in the Tibial Slope Reduces Wear after Medial Unicompartmental Fixed-Bearing Arthroplasty of the Knee

Introduction. Unicompartmental arthroplasty of the knee in patients with isolated medial osteoarthritis gives good results, but survival is inferior to that of total knee prosthesis. Knees may fail because positioning of the prosthesis has been suboptimal. The aim of this study was to investigate the influence of the tibial slope on the rate of wear of a medial fixed-bearing unicompartmental kn...

Comparison of Wear in Fixed and Mobile Bearing Knee Designs

Ultra High Molecular Weight Polyethylene (UHMWPE) remains the only bearing surface of choice in total knee replacements (TKR). While excellent medium term results can be found for knee replacements at ten years, there remains concerns about wear debris induced osteolysis in the longer term, particularly in young and more active patients. It has been shown previously that the wear of fixed beari...

Wear Comparison of Fixed vs. Mobile Bearing Total Knee Replacements

INTRODUCTION: The debate between fixed-bearing and mobile-bearing systems continues due to the lack of clinical evidence showing survivorship of one to be superior to the other [1,2]. Delport et al report similar tibiofemoral kinematic patterns with respect to axial rotation during flexion and also femorotibial translation [3]. Conversely, Huang et al show a significant difference between the t...
